As your Aging Life Care Specialist, I will create a Care Plan based on the strengths, needs, and goals of both caregivers and of the older adults in the family.

This Care Plan may include:

* Care Management

* Providing assessments of physical, cognitive, and psycho-social functioning of older adults

* Help with finding reliable home care

* Assist with moving to long term care facility

*Help with understanding palliative care

*Assist with understanding the costs and eligibility for various Medicare/Medicaid programs

* Facilitate family meetings

* Supervision of in home staff

* Assistance with medical appointments.

* Advocacy

* Communication with family members

* Monitoring home care

* Medication management

* Education

* Health promotion
